Diastasis recti, also known as abdominal separation or postpartum belly, is a common condition that plagues about 60% of pregnant women and at least 30% of women 12 months postpartum. It is characterised by the complete or partial separation of the rectus abdominis muscles– the six-pack muscle. 

The rectus abdominis muscles are divided into the left and right sides with a band of tissues called the linea alba running down the middle. During pregnancy, the uterus, muscles, and skin expand to accommodate the growing foetus. This causes the linea alba to stretch and thin, separating the two sides of the rectus abdominis muscles.

Diastasis recti occur when the linea alba expands and thins during pregnancy.

There are many different types of diastasis recti, depending on where the separation occurs along the rectus abdominis. Types of diastasis recti include:

Although diastasis recti most commonly occur in women, men, and babies can also be affected. 

What causes diastasis recti?

Diastasis recti occur due to internal abdominal pressure. This pressure puts excessive strain and stress on the linea alba causing it to stretch and thin out. In some individuals, the linea alba gets overstretched and does not return to normal, resulting in diastasis recti. 

Causes for this internal or inner-abdominal pressure include:

  • Pregnancy
  • Lifting heavy weights incorrectly
  • Excessive, unsafe abdominal exercise

What are the symptoms of diastasis recti?

Symptoms of diastasis recti include:

  • Soft bulge or pooch just above your or below your belly button
  • Pain during sex
  • Pelvic or hip pain
  • Bad posture
  • Bloating
  • Constipation

Is diastasis recti painful?

Diastasis recti or the separation of your ab muscles is not painful by itself. However, you may experience issues such as poor posture which may give rise to pain.

Diastasis recti can result in weakness of your core muscles, thus leading to hip and pelvic pain, pain during sex, and lower back pain.

Who is at risk of diastasis recti in Singapore?

Anyone can be affected by diastasis recti, however, there are some factors that can increase your risk of developing this condition.

Risk factors include:

  • Being pregnant over the age of 35
  • Pregnant with multiple babies, e.g., twins, triplets
  • Back-to-back pregnancies
  • Pregnant with a big or heavy baby
  • Having a vaginal delivery
  • Being petite or small-sized

Some individuals have a higher risk of developing diastasis recti than others.

How is diastasis recti diagnosed?

Diastasis recti are usually diagnosed using a physical examination of your abdomen. 

You may be asked to perform the following self-check to identify if you have diastasis recti:

  • Lie on your back, bend your knees, and place your feet flat on the floor.
  • Lift your shoulders and head off the floor and tuck your chin into your chest.
  • Place one arm behind your head as support. 
  • Place your other hand above your stomach and with your fingers, press down on your abdomen just above your belly button.
  • Use your fingers to feel for any gap or separation between your left and right abdominal muscles.
  • If you can fit 3 or more fingers in the gap or separation, you have diastasis recti.
